Home personal care supports various personal hygiene and daily living activities, allowing seniors to age safely and independently. Our caregivers can provide on-call assistance, non-stop care, or anything in between.

Bathroom and toilet assistance, meal preparation, light housekeeping, and similar services are all examples of personal home care. Our caregivers can also help seniors with cognitive impairments, those recovering from illness or injury, or those at risk of falling.

CAP/C

CAP/DA

 CAP/C Services (Community Alternatives Program for Children)  CAP/C is a Medicaid Home and Community-Based Services (HCBS) program that provides eligible children with alternatives to institutional care. Through this program, families can access personalized services designed to enhance their child's quality of life while promoting independence and family involvement. 

 

Medically fragile and medically complex children who are age 0 through 20 years of age


 hands-on assistance with activities of daily living (ADL) and instrumental activities of daily living (IADL) tasks. 


 Temporary relief to the primary unpaid caregiver(s) by taking over the care needs of the waiver participant for a limited time.

Community Alternatives Program for Disabled Adults 


CAP/DA is a Medicaid Home- and Community-Based Services   

The waiver program provides a cost-effective alternative to institutionalization for a Medicaid beneficiary who is medically fragile and at risk for institutionalization if home- and community-bases services approved in the CAP/DA waiver were not available. CAP/DA services allow the beneficiary to remain in or return to a home- and community-based setting by supplementing, rather than replacing, the formal and informal services and supports already available to an approved Medicaid beneficiary.
